Since the distribution was a qualified distribution, TurboTax doesn't really need to know your Roth IRA contribution and conversion basis. You can skip those questions. Just make sure to indicate to TurboTax that you had a Roth IRA before 2014 so that TurboTax will treat any code T Forms 1099-R the same as code Q Forms 1099-R, as qualified distributions that are not reportable on Form 8606 Part III.
