The Form 1099-R is correct.
Assuming that you are the owner of this IRA and this is not a beneficiary IRA, the amount redeposited constituted a rollover. Enter the Form 1099-R exactly as received and in the follow-up indicate that you moved some or all of the money to another retirement account or back to the same account, indicate that you rolled over part of the money, then enter the amount redeposited. The gross amount distributed will be include on form 1040 line 15a or Form 1040A line 11a but the amount rolled over will be excluded from the taxable amount shown on line 15b or 11b. The word ROLLOVER will appear next to the line.
