How do I enter 1099-R info manually?

I'm trying to enter 1099-R info for my Johnson & Johnson Pension Plan. Turbo Tax can't find the Financial institution. How can I enter it manually?

You will see a box in the lower right of the screen: Change how I enter my form.

Click that box and you will then see: Type it in myself

Then, click the appropriate type and hit continue. You can then type in the information.
