How can I report a loan for 401k? I do not have any form.

The 401k gave me a loan of $3,000 but people told me that I need to report this when I do my taxes but I dont have the forms and I don't know how to do it. I am paying that account and they take money from my check every paycheck. Do I have report it when I do the taxes?
