Does Form 480.7C from 2020 Retirement Plan of Puerto Rico is the correct one I should use to fill out my tax Return to be able to receive the $600 that I didn't receive?

I received the 1st $1200., not the 2nd $600 but did received the 3rd of $1400. Both direct deposit. My  monetary situation hasn't change.
