Do I report my Canada Survivor pension as CAD or convert it to USD?

You should convert it to USD using an average exchange rate for the year. A good resource to find this is on OANDA which you can link to, here:

You may also want to look into the USA-Canada treaty because depending on where you reside, the pension may not be taxable.
