Do I report a Simple contribution on line 28/1040 if it was excluded from my wages on line 1 on my W2 and shows as a retirement plan contribution on line 12A of my W2?

No, you enter the W2 as its shown.  You don't enter those figures anywhere else. Its already been accounted for on the W2.
