Code W means Charges/payments for purchasing qualified long-term care insurance contracts under combined arrangements.

 

You don't have to report this form 1099-R on your tax return. Just keep the form with your tax records for 3 years.

Please clarify. What is the code in box 7 of your form 1099-R?
