Is it an ESOP plan? For an ESOP, when you closed out your retirement account, your stock would be sold back to the company. The company would have issued you a 1099B for the stock sale. Check your 1099R. If you have any amount in box 6, then your plan was probably an ESOP and you should be recording the sale of the shares back to the company. If you do not have a 1099B, check with your company HR or whoever administers the plan. That is unless you keep the shares, then you would not have a 1099B form.
