amended return using form 4852

I have to file an amended return since I had an incorrect 1099R from an annuity company. The company refuses to correct the 1099R so I have to file a form 4852. How do I do this in TT? Do I put the correct figures in on the 1099R and check the box that states this is a corrected 1099R? Obviously I will have to include the form 4852 when I submit my corrected return.
