You get the information (amounts) for 1099-R boxes 8 through 11 and 12 through 14 from the 1099-R form. The 1099-R form is prepared by the Payer. If your 1099-R doesn't have information in boxes 8 through 11 and 12 through 14, you can leave those boxes blank when entering the 1099-R in TurboTax.
