Box 11 is for the first year you made ROTH IRA contributions. That is significant since distributions from a ROTH IRA from contributions not held in the plan for five years may be subject to tax and penalties on the income earned.  I suggest you enter a year that will give you the proper five year holding period based on your best estimate of when you first funded the plan.

 

I don't see a box 13 entry in TurboTax for a Form 1099-R, but on the form from the IRS it says the date of payment, which would be the date you received the distribution.
