FBAR - The owner of the account is NON US Citizen

I'm a US citizen and co-own the foreign account in Israel with my dad. I have no financial interest in this account, never use it and have a signature authority only if something g-d forbid happens to my dad (he is 87 years old). He is an Israeli citizen, does not reside and has no address in the US. This year the maximum balance went slightly over 10k and I believe I have file an FBAR because of that filling out Part IV.

1. Since my dad is not a US citizen, what do I put in place of TIN under #35 . He has none (no SSN, no TIN or any US related numbers)

2. Do I have to fill out his address?
