On the 1099-B, when entering the sale of stock that has one purchase date, but various sale dates all qualifying for Long-Term , do I have list each individual sale?

If the stocks are all in the same Box D category, you do not need to enter each sale and you don't need to mail any support information. If they are in different boxes, you will have to enter each Box category separately. Box E & F entries require you to mail support information. I'm not sure what happens if you have adjustments in box 1f, 1g or other adjustments.
