Incentive Shares repurchased, not reported on w-2

I had incentive shares repurchased in 2019. This was all within the company, not an outside brokerage. The proceeds were not included on my w-2. Should I receive a different form with the proceeds recorded?

In addition, an 83b was filed at offering, but only 75% were vested at the sell. How will that impact me?
