I bought MRK stock for that spun off MHS stock that then merged with ESRX. I sold ESRX in 2018 and 1099B shows code x in block 5. How do I report my cost basis in TT?

You simply enter it in the "Cost or other basis" box of TurboTax's 1099-B entry form, just like investors have been doing for decades and decades before the IRS dreamed up the notion of "covered" and "noncovered" securities.
