Direct Transfer of Individual Account

I did an institution to institution transfer of one of my individual accounts. The amount that was transferred over were considered as income in 1099-B, how do I fix this?

Direct Transfer of Individual Account

contact the institution to find out why.  the IRS has a 1099-B, so there will be issues if you don't report what's on it
