1099b worksheet

My 1099-B form is incomplete - the majority of shares sold were purchased over a period of several years and there is no information provided to complete the necessary tax forms.

1099b worksheet

Your 1099-B is not "incomplete". Box E is checked indicating the broker is not required to report the cost basis to the IRS.

You are expected to have that information in your own records. Even though the 1099-B does not show the info, you broker may or may not actually have it. You may enter the word "various" for the acquisition date.
