If I refinanced with different lender, and original lender's form 1098 bx 5 is blank, but disbursement activity reflects PMI, can I enter that amount?

Yes, you can enter the amount that was disbursed for PMI.
The 1098 may have been issued before the recent tax law change that allows the amount to be deducted for 2017.
