What category should I choose for LTDS-T and MHA on my W-2? Thanks.

You can simply use category Other to report your LTDS-T (Long Term Disability?) and MHA (Minister's Housing Allowance?) contributions if there is no corresponding category.

Enter the description from your W-2's box 14 on the first field in the row. Enter the dollar amount and select the correct tax category that goes with that description.

The software will help determine if this impacts your tax return.
