attachment For 8453

I signed the attachment form 8453 for corporation signature.  I see it is an attachment in tools/attachments but when I print the return, the form 8453 comes out blank and the signed copy is not included.  How can I make sure the attachment is included when efiling?

The signed copy will be included as long as you uploaded it. The file is a separate file on your computer.
