Why is it being taken out of my refund. 12a code I was told to use it on medical only

Please clarify your question by adding some  additional information so we can provide a more helpful answer.  Are you entering a Form W-2?  What code do you have in Box 12a?  What do you mean by you were told to use it "on medical only"?
