I had a ddd in where's my refund but that date has come and gone now it says theres a delay in processing and my refund date May change. Error 1121. What's going on

It is not an error code.  Rather, code 1121 means that your tax return is being reviewed.

The automated system has flagged something as being out of the ordinary and your return is being queued for review by a human. This may or may not result in further investigation or challenge by the IRS. All you can do is wait. The IRS does not notify TT.

I had it direct deposited into my girlfriend's account.  Im wondering if that has something to do with it and her bank may have rejected the money since it's not the same name in account

Well when the IRS does release the refund expect the bank to reject it (UNLESS you can get your name on the account before that time) and the check be sent back to the IRS who will convert it to a paper check mailed to the address on the return.
