No mailing address for New York forms

I have to 'Print and Mail' one form to New York (after e-filing my return) but no mailing address or instructions included -- instead the standard summary page was included (seems like a bug).

I did find an address for the general State Processing Center but seems unlikely it will get to the right place without a department or anything...any ideas?

Other details: I e-filed using Desktop for Mac, everything else worked fine. Fwiw, the form is the Metropolitan Commuter Transportation 2019 Mobility Tax Worksheet (MCTMT)
