Letter 12c from IRS Please Help

I receiver a letter 12c.

It is requesting

 

-W-2-no problem got it

 

-Schedule 2, incomplete or missing. What do I send? the schedule 2 an SE?

 

-Schedule 3, incomplete or missing. I did discover that I made an error. I have a 1095-C healthcare covered by employer. I will send a copy of that. Is that the correct way to respond? Do I need to do or include anything else?

 

-Schedule 1, incomplete or missing. Send Schedule 1 and schedule C?

 

filed joint my husband has W-2 I am self employed.

do not send the IRS 1095-C it has no effect on your return and is not reported. 

look at schedule 3 and see if there are amounts on any lines - send schedule 3 + any form or schedule for any line with amounts.

note we can not see your return - schedule 3 is for payments and credits  which include all of the 

here's what can appear on schedule 3

foreign tax credit form 1116 send the form only if over $600

child and dependent care credit form 2441

education credit form 8863

retirement savings credit form 8880

residential energy credit form 5695

forms for any other nonrefundable credits

estimated tax payments - no form

net premium tax credit - (healthcare.gov coverage) form 8962 and form 1095-A

extension payment - no form

certain other refundable credits.
