I have to print out and mail my state return for Illinois. Do I have to put in anything else except my signature before mailing? For example, PTIN.

You only need to sign and date the return. The PTIN box is for professional tax preparers. Please check the instructions on the first page of the return. You may need to attach copies of your W-2 and other tax forms to the tax return.
