Do I report when an Employer 401K is terminated and rolled directly to a brokerage Traditional IRA?

Husband was employed for 15 years at a company and had a 401 K that was terminated and rolled directly into a brokerage traditional IRA. Is there any need to report this?

Do I report when an Employer 401K is terminated and rolled directly to a brokerage Traditional IRA?

You should have received a 1099-R form for the rollover. This would not be a taxable event but it is reportable on your tax return. Did the rollover take place in 2017 or 2018? If 2017, then contact the 401k custodian and inquire about the form.
