Form 1040V - Do I need anything other than that and the check?

If paying in monthly installments to the IRS, do I need to include anything else with form 1040-V to make my initial payment?  The form I got from TurboTax says to use the voucher with the 1040, their website says not to include a copy of it.  This whole process is very confusing.

The IRS takes 30 days minimum to approve an installment agreement request, so yes mail or pay directly on the IRS website your initial payment.
