I am a 1099 contractor this year in Arizona.
Do I have to pay the self-employment tax and the estimated income taxes both quarterly and if so, can they be paid together or are there separate forms for each one?
Also, do these have to be paid on a state level too?

Yes, it would be advisable to pay estimated taxes as a self employed contractor.
For self-employment net profit keep in mind that there are two parts to the tax, self employment tax and personal income tax.
