1099 form for my 2019 federal taxes

IRS sent me a letter last month requesting I pay over $3K for taxes in 2019.  I requested an extension and must have help finding the information needed

I earn income from a private music studio in my home.

 

I have been and continue to be a church organist.  In 2019 my church employment changed from an employee to that of a private contractor with a monthly income that was always, and continues to be $550 every month.

 

Please help me know how to locate the information and paperwork needed from my 2019 income tax forms

1099 form for my 2019 federal taxes

You would have a 2019 Form W2 for the wages you received while an employee.  

 

You would also have been issued a 2019 Form 1099MISC for the amounts paid to you while an independent contractor.

 

Review your filed 2019 tax return to determine which of the two types of income was reported on your tax return.  If you reported your wages from a Form W2, then you need to report the income while an independent contractor, a Schedule C.  In TurboTax you can enter your self-employed income and expenses, and the Schedule C will be generated to be included on your amended tax return, Form 1040X.
