I’m using desktop, so I click Forms, select the form and then hit delete form. It just comes back immediately

I have none of these and none are populated on the form. When I try to delete the form it just adds it back

I also have this 8995-A, but nothing in box 5 of my 1099-Div. I cannot delete the form. What else could be causing this to generate?
