IRS Form 4506-T

I am getting ready to file form 4506-T to request a couple of old tax transcripts.  

Line 3 - says “current name, address ( including apt., room, suite, or inmate no.) city, state and Zip code.”

Do I enter my name again and enter current address info or just my current address ?  I am not incarcerated

Line 4 - says “previous address shown on the last return filed if different from line 3”

On the transcript I am requesting the address was different from our current address. So do I put the address that was show on the tax return for the year I am requesting the transcript for. 

 

I am trying to get this submitted correctly as I don’t want it to be rejected.

    Line 3 is for where you currently reside and where you receive your mail

     

    Line 4 is for entering the address from the tax return that you submitted if it is different than your current address.

    Thank you for your information on this … Is there a need to put my name again on line 3 or just my current address?

    You can just put your address on Line 3. I have seen it done both ways, though, with the addition of the same name that was on Line 1a. I really don’t believe it makes any difference.

    I am sorry but just want to make 100% sure I understand …. Line 4 is for the address of the  return transcript I am requesting, which is different from my current address.   Correct ?

    Line 4 is for the address on your previously filed (most recent) return IF different than the address on Line 3.

    I am sorry, I am having a hard time understanding  - again I am just trying to do this correctly …

    On the form 4506-T I am requesting 2018 tax return transcript … 

    When the 2018 tax return was filed we lived at XYZ .  In 2022 we moved to ABC which is now our current address.  All tax returns from 2022 forward have had the ABC address. 

    So I guess what I am asking is do I use the address from the 2018 tax return or do I use my current address.

    Your current address if that’s what you used on your last filed return. The IRS wants to know if your address changed since you last filed.

    This should not be difficult -

    You want the IRS to mail you a transcript of a previously filed federal tax return.  For you to receive the mailed transcript you need to tell the IRS your current mailing address (Line 3)

    The IRS needs to know the address that was on the previously filed tax return (Line 4 - if different than Line 3)

     

    If your name is the same on the previously filed tax return (Line 1), there is really no need for you to enter your name again (Line 3)
