What type of 1099-R   regular 1099-R , CSA 1099-R, RRB, 1099-R ?

 

What code is in box 7?

What is in box 2a on your 1099-R?

    form 1099-r  and 7d


     sorry

     

    I took out an annuities back in the mid to late 1980's

     

    and getting ready for retirement so I cashed in to pay off credit cards and some loans

     

    I got 33759.03 line 1

    18443.32      line 2a

    15315.71       line 5

    7D                    line 7

     

    but cannot find where to enter all the lines , it just lets me enter the total for line 1
