I received a email saying I needed to mail in an extra form for my state refund and I do not know what form it would be?

If you e-filed your state return and where suppose to mail anything in you can find what it is that you need to mail by opening up the PDF version of your tax return, before printing, and you will see an instruction page with what you need to print and mail. I have provided instructions below on how to download and print the PDF version of your tax return:   

1. Sign in to your account (if you haven't already done so), using the same login you used to file your return.
2. If you can't remember, go here so we can help you get logged in.
3. Once you're logged in, go to the bottom of the screen and click My Tax Returns and Documents. 
4. Choose the tax year for the return you want to print
5. Select Download/print return (PDF).
6. On the next screen, click Download my return.
7. Once your return has downloaded, locate the PDF and open it. (Once opened you should find the instruction page if you are required to mail anything in) If you see a printer icon at the top of the screen, click that; or if you have a File menu in the upper left corner, open that and select Print.
8. Make any adjustments in the Print window, such as the number of copies, and then click the Print button at the bottom.

I received an email exactly like this as well, but when I printed out my PDF version of my 2020 tax return, there were no instructions or additional details as to what forms I needed to mail in for my State tax return.  Any advice would be greatly appreciated.

Same exact issue. I also got an email saying my state accepted my return but that I have an additional form that needs to be mailed in. I followed the instructions from the email and this post, but when I download the PDF copy of my 2020 return and/or print it, it does NOT contain any pages with additional instructions about what to print. How do we know which form needs to be printed and mailed to our state??

What State is this for?

I have the same issue. My email says extra form needed after my state was already accepted. But the pdf doesnt show any instruction on what i need to send. 

@Shawnatyn  Unfortunately, in this Public Forum we don't have access to your return information.  Your best option is to contact your state's taxation department at the phone number they provided on your notification.  Write down the information they provide, and if you can provide us with the name of the form in question, we can provide additional assistance on how to find the form they are asking for.
