Where can I get the refund request form? An agent said she'd email it to me but never did, and that was last week.

Since you have already paid, please feel free to contact Customer Support so they can assist you. You will want to use the keywords billing issues when you call so you get to the right person.

I was triple billed.1 refund in checking account.So I’m still being double charged
