I can not find any help with a W-4S form for withholding from sick pay 2018. How can I find out how much I should be withholding from my monthly benefit?

No, I have the form but can't figure out how to fill it out. It's line 4 (figuring my tax, rate schedule) that's stumping me

What is your filing status and what is the amount you entered on Line 3 of the W-4S?

From the Form W-4S you would use Schedule Y-1 -
The tax to be reported on Line 4 of the Form W-4S is $1,905 plus 12% of the amount over $19,050
$24,216 (-) $19,050 = $5,166
$5,166 x 0.12 = $619.92

Tax reported on Line 4 - $1,905 + $619.92 - $2,524.92 (rounding up) $2,525

@Tasha11284 Assuming that you are not itemizing deductions on Schedule A then Line 1 and Line 3 of W-4S would be the same amount.

What is the amount on Line 3 and what is your Filing Status?

Single

Married Filing Jointly 

Married Filing Separately

Head of Household

@DoninGA @Single. I think it's 5,950 if I am doing it right. I have been only able to work part time since a severe car accident so gross earnings are lower this past year.

@Tasha11284 I should have written $595 to be entered on Line 4 of the W-4S.  From there you can calculate how much should be withheld from each payment by following the instructions up through Line 10.

@DoninGA  ok, so I take that $595 and divide it by how many payments I'm expected to get? I'm sorry if these are dumb questions lol but looking at the worksheet it doesn't compute to me lol

Correct.  Divide $595 by the number of payments you expect to receive.  That would be amount to enter in the box of the Form W-4S labeled - 

I request federal income tax withholding from my sick pay payments. I want the following amount to be withheld from each payment.

@DoninGA I think I figured it out. My other question is, my leave starts the end of January so will these numbers change starting a new year? I will also be on reduced hours 3 months after the Paid Leave as well. The numbers I am filling out are for 2022 but the leave isn't until Jan 2023.

@Tasha11284 The W-4S you will be completing is for tax year 2023 so if you expect that your Adjusted Gross Income in 2023 is correct at $5,950 the number calculated is correct for the taxes to be withheld.

@DoninGA   I was giving the numbers for expected 2022 income, I don't even have an guestimate what 2023 will be with how long this leave may have me out past Paid benefits. Thank you for your help though. I will go with the numbers I worked out for the forms Paid Leave provided me.  I hope I can always adjust it if needed!

Yes, you can always submit a new W-4S if you need to adjust either up or down on the taxes being withheld.

@DoninGA perfect, and I'm sorry, just one more question, when we say federal taxes I want taken out, does this include the coverage for Medicare, SS etc? Are those grouped into that tax ? Viewing some of my pay stubs it looks like they're taking out about $60 a week for federal, social, Medicare, paid leave taxes all combined.

This is Only for federal income taxes to be withheld from the sick pay.

@DoninGA ok thank you. Any idea why it would be so high for just federal-looking at $49 per payment, if no other taxes are being taken out?

@DoninGA Can you assist me with this as well? I am married filing jointly. Based on my initial calculations, I have to withhold more than the CT paid leave benefit check would be. The paid leave benefits are far less than my normal salary. How would I handle this on the form? I can't find any support online for this sick pay form specifically.

Can you be a little more specific with your question? I'm not sure I understand what you are asking?
