I recently retired and started receiving distributions from a Cash Balance account. It was an annuity purchased by a previous employer. I did not contribute to it - it's my pension. Is it considered Qualified of Non-Qualified?

It is usually qualified so let me ask you to check a few boxes to be sure.
Look at box 1 and box 2, in a qualified plan, they will match. Box 5 should be zero since you had no employee contributions
