I was supposed to have direct deposit but received a check from Civista Bank with an additional $20 taken out, has anyone else had this happen?

They couldn't Direct Deposit it.  Your bank account number or routing number was wrong so the bank rejected the deposit.  So they had to mail you a check and they charge $20 to mail out a check.  

Do you have a state refund?  Did you get it yet?  They will probably have to mail you a check too.  Check with your bank for the right numbers to use.  Although it's too late for this year.

Thanks for your response, I tried looking up the bank and was not able to find anything. I'm very skeptical about depositing it.

Oh, it's ok.  That is the bank your refund first goes through to have the Turbo Tax fees deducted from it.

I checked my account number & routing number and they are correct on my tax form.

Check with your bank ... is the numbers correct for a direct deposit ... what is on your check or deposit slips are not always the correct numbers.

Did you ask the bank why they rejected the deposit?  Is your name on the account?  Some banks use different numbers than the ones on the bottom of your checks.  Like Bank of America.

I used my girlfriends bank bc i owed her some money. The check had my name on it not hers.

That's why.  Her bank rejected the deposit because your name is not on her bank account.

Banking regulations forbids  direct deposits to accounts where your name is not on it ... this is an anti-fraud, anti-laundering  regulation.
