How may I file form 941 to make my quarterly Social Security tax payments as an independent contractor?

I am being required to show proof of paying the previous quarter to continue with my medical benefits in AZ .  I just became an independent contractor doing respite care.  My 1st pay as a contractor was in Sept 23

You don't file 941 or W2 for yourself.   You just pay quarterly estimated taxes on form 1040ES.

How to make the Estimated payments

Here are the blank Estimates and instructions…..

Or you can pay on the IRS website. Be sure to pick 2023 1040ES payment

Independent contractors make their Social Security payments via the SE (Self-employment) tax.  See this IRS reference for a detailed explanation:

https://www.irs.gov/businesses/small-businesses-self-employed/self-employment-tax-social-security-and-medicare-taxes

Independent contractors may have to make quarterly estimated tax payments, because taxes are not withheld from payments made to them.  Those estimated tax payments can be used to pay the self-employment tax.

The Self-employment tax is reported on Schedule SE, which is attached to your 1040 personal tax return.
