Can I use Form 4684 with Turbo Tax get credit for car damage expense from a tornado that my car insurance didn't cover?

@DoninGA My question was meant to ask WHERE to enter Form 4684 on TurboTax, not requirements for its use because the disaster I am inquiring about was, indeed, a federally declared disaster. That said, I believe I have found an answer to my question after re-wording my question.
