Normally errors on the AZ form 322 are related to the school codes or the code not matching the school or district name.   It may be helpful to delete the form and re-enter the information.

In TurboTax desktop versions use the "Forms View" to find and delete the AZ form 322.

In TurboTax online versions:

  1. "Tax Tools" in the left hand menu
  2. "Tools"
  3. "Delete a form" in the main screen
  4. Scroll/find the AZ form 322
  5. "Delete"
  6. "Confirm Deletion"

  1. Revisit the AZ state interview and click through to the credits section.
  2. The first option is for donations actually in 2021, if you made the donation in 2022 enter in the next section
  3. If the boxes don't apply be sure there are no stray spaces by deleting/backspace as needed to empty the box
  4. Be sure the school code used is fully entered; it must be a nine-digit code
  5. Use the school name and district name that match the school code on the list you are pulling from

Here is a link to the school code list for form 322 if needed.

On Arizona form 322, TurboTax indicates an error "Contribution date must be entered." I contributed before April 15, 2023 and filled in the correct 2023 date. TurboTax demands the 2022 date field also be filled. I did not contribute in 2022.

Please try the steps above mentioned by DMarkM1 to delete form 322 and add the information back to see if this resolves your matter.  If it does not, please post back here in the thread for further assistance.

I did delete the form and started a new one. Same problem. The workaround ended up being put the zero in the amount and filling in a bogus date. A date field was still required to stop the error message.

Had a similar error issue - usually fixed by going to the "forms" view and deleting the "0.0" so that program stops thinking money donated in that time frame and stops asking for a date.
