I know the amount of my retirement funds in 2020, but I do not know the information required for prudential (the payer)?

Federal ID number, address, etc.

How do I file a substitute W-2 using Form 4852?It is best to contact Prudential directly. They should have issued a Form 1099-R to you.

 

Tips to locate your Form 1099-R

• Check your provider’s online portal for it.
• Contact your plan administrator to get your form.

If you don't receive a form after requesting one, you can still file your return by completing a substitute Form 1099-R, following these steps:

1. Inside TurboTax, in the search box, enter 1099-r and select the Jump to link in the search results.
2. Follow through the prompts; You will need to select I'll type it in myself.
3. Skip the address information for now and Continue.
4. Answer the questions as best you can.
5. At Do any of these situations apply to you? select I need to file a substitute 1099-R.
