I got an IRS notice. Asking for 'Statement of benefits on retirement income indicating the date issued, gross amount, taxable amount and withholding'. Where can I get it?

If you received retirement income then you should also have a 1099R from the custodial of the retirement account.   If you did not receive it, or if you misplaced it, you need to seek a copy of it from the custodial of the retirement account--typically a bank or brokerage, etc.

I am only 40 years old. I haven't got any retirement income.
In this case, what should I reply?
If I need to seek a letter from 'custodial of the retirement account' saying I didn't any retirement income, should I check with the bank I hold account with?

First make sure it is really from the IRS.  Might be a scam.  Did you take any IRA or 401K withdrawals?  Or do a rollover or conversion?  Probably ask the IRS why they think you got retirement income.

I thnk it is legit. IRS has held my tax refund for 2021. I got the notice (CP05A) from IRS asking for three details for reviewing my tax return. I have first two. which is my payslips and detail about my employer.

I don't have information for  the 3rd question, Which I stated above. Wondering what to reply them.

I most commonly see the CP05A for a military retiree that has qualified for VA disability benefits retroactively. Is this your case? You did state the notice is asking for "statement of benefits on retirement income". At 40 years of age it is perfectly possible for you to retire with 20-22 years of military service and qualify for the retirement pay.  I myself retired at the age of 38 with a 10% service connected disability that exempted 10% of my retirement pay from taxation.

All you can do is call the IRS and ask why you got the letter and what do they want.
