Entering information for 1099-SA form who's data I need to enter for Payer Name and address?

You are in the wrong place.  There is no payer to enter.

 Go to Federal> Wages & Income>>Retirement Plans and Social Security  (SSA1099 and 1099RRB) to enter your SSA1099.
