Does it matter if I submitted a mock 1099-R that errantly has an entry in box 14? I made a mock 2018 1099-R for my 2017 return. On my actual 2018 1099-R box 14 is blank.

I made an excess Roth IRA contribution in 2017 and removed it in 2018 prior to the tax due date. Per TT advice I made a mock 2018 1099-R. On this mock 1099-R I have MO(for Missouri) listed in box 13 and in box 14 I have $99 which is the same as in box 2a. All other information is correct.

On my real 2018 1099-R boxes 13 and 14 are blank and there is 0.00 in 4 and 12.

I cannot recall if I put the information in my mock boxes 13 and 14 or Turbotax auto-filled this. On the pages following the 1099 section TT did ask me where I lived and the source of this income and actually has a spot for entering a "Missouri Distribution Amount". I believe this is due to TT trying to see if I qualified for a Missouri tax exemption due to this being a private pension.

Lastly, form MO-A seems to be the only Missouri form that uses this information. It is in Part 3 Section B where they try to find a private pension exemption.

Long story short - is it an issue that I have something in box 14 when I should not?

Leave box 4 and 12-14 blank - not zero.

4 and 12 were left blank. I have MO in 13 and the value of 2a also in 14. This was all already submitted and accepted on my 2017 return. My concern is if I need to amend my 2017 return and have blanks in 13 and 14 OR if this is not an issue.

If you already filed, it is a non issue.   It only matters when you try to e-file a 2018 tax return.   It is a 2018 TurboTax e-file issue and makes no difference for an amended 2017 tax return that can only be mailed.

No, it will not be an issue if you have the same amount in Box 14 (State Distribution Amount) as in Box 2a (Taxable Amount), as it is probably also the same amount as in Box 1 (Distribution Amount).

Even though Box 14 may be blank on your original 1099-R, if the state taxes the 1099-R income, it automatically transfers the Taxable Amount from 2a. 

This is often left blank on 1099-R's as it is not known by the Plan Administrator if the state will tax all, part, or none of the distribution for a particular taxpayer.

My box 1 is not the same as my box 2a. This was a Roth distribution for excess contribution so 2a is just the earnings.

So 14 matches 2a but not 1. Is that ok?

This was all on my 2017 return which was accepted by the state I work in, the state I live in-MO, and the fed. I submitted a mock 2018 1099-R on that return—I selected 2018 when TurboTax gave me the option on my 2017 return to pick the year of my 1099.  

Is this related to TurboTax asking me what state I live in after filling out the 1099? Is this just how Missouri does it?

Don't worry about it.   Box 13 and 14 ONLY matter if there is a positive dollar amount in box 12 - if not then what is in box 13 & 14 only matters for the TurboTax e-file error check and makes no difference for an 2017 tax return.     Neither the IRS nor state even look at box 13 & 14 of box 12 has not tax withheld.
