State and Fed rejected ACH payments, state charged fee, reason makes no sense

Both the state and the IRS rejected my ACH payments, but the routing number and account number are valid (the same account I have used for years).

 

The state charged me a $25 "returned payment fee".  I have since paid the $73 to the IRS and the $42 to the state.

My bad.  I had forgotten that after someone forged a check against that account a few months ago, the bank had changed the account number.
