Question filling out IRA distributions, annuities on 1040 form

I’m filing my 1040 electronically. Box 4a and 4b on the 1040 form asks for “IRA distributions” — Do I just add up all of my distributions and enter the amount on the 1040 form? — or is there some additional form on which I can write down each distribution and then add them up?

No, there is no form, line 4 on Form 1040 comes directly from the IRA Forms 1099-R you entered. Forms 1099-R for pensions and annuities will be on line 5.
