How were my turbo tax fees paid if I my refund hasnt been approved?

Your fees have not been paid.  The order says it is complete because you signed an agreement that says your fees will be paid by your refund OR if the IRS takes unusually long to process the return, TT can take the money out of your account.    So TT knows it will be paid one way or the other.  

When you efile your tax return successfully and you choose to pay your fees from your Federal refund, the TurboTax program considers the fee paid, even though the federal refund has not been released yet. This is why you see "Payment Completed".
