Will Turbo Tax provide me with the Identity Theft Affidavit form 14039 or do I have to get that myself from the IRS website and send in with my returen?

Yes, you will need to download Form 14039 Identity Theft Affidavit from the IRS website.  TurboTax does not support that form. 

 

Below are two links.  The first is the link to the IRS Form 14030, a pdf file.  It is a fillable form and while you can save it as a pdf file to your desktop, if you save it you probably will not be able to open it again and continue to enter information as you had done previously.  Therefore, we recommend that when you start to prepare the form, you complete it at one time.  When you are done, save the form to your desktop, print it out, and forward to the IRS.  You can fax the form to the IRS if you choose (see page 2 of the form for facsimile instructions).

Is this answer still current? I am using TurboTax desktop deluxe 2021, and it asked me information about the identity theft. When I print out the recommended version of the returns for records, the form is there with the information I entered. So just to make sure, as of 4/3/2022, does the ID theft affidavit form get e-filed with the rest of my return or must I send that form separately through the mail?

Here is the federal forms list of forms that are available in TurboTax. Form 14039 is available but unsupported. It must be sent separately through the mail.
