Where is Line 1 in Box "D" on my W2 I only see the Control Number?

Turbo Tax is asking me for information from line 1 in Box "D" on my W2 and the only information there is the Control Number. Where is line 1in Box "D"?

The Control Number in box d of the W-2 is not required to e-file your tax return.  Leave box d blank (empty).  If the program still gives an error then delete the W-2 and re-enter manually to clear the error.
